Plaintiff ouizeb ("ouizeb" or "Plaintiff"), by and through counsel, submits this motion for
entry of default.
1. DEMAND - Punitive. $8000 in damages from Defendant GiocomoCatuara
regarding Counts One and Two.
2. DEMAND - Declaratory Judgment. Declaratory Judgement that Defendant
GiocomoCatuara assaulted Plaintiff ouizeb.
3. DEMAND - Declaratory Judgment. Declaratory Judgement that Defendant
GiocomoCatuara battered Plaintiff ouizeb.
4. DEMAND - Fees. $125 in fees of the Plaintiff filing this civil action against
Defendant GiocomoCatuara
5. DEMAND - Fees. $2700 in attorney fees.

AFFIDAVIT
I, Sawyeriez, do hereby swear under the pains and penalties of perjury that the statement
contained within this affidavit is, to the best of my knowledge, the truth, the whole truth and
nothing but the truth.
1. I am of sound mind and capable of making this statement. I understand if I lie in
this statement, I may be criminally liable. This statement is accurate to the best of
my knowledge and belief.
2. On the 28th of July, 2024, Plaintiff filed suit against Defendant GiocomoCatuara.
On the 30th of July, 2024, counsel for the Plaintiff submitted their civil complaint.
3. Defendant was served with a summons on the 31st of July, 2024 by Sheriff Deputy
Just_Dapper.
4. A response to the complaint was due on the 6th of August 2024 - as set by His
Honor ArthurSprings.
5. Defendant GiocomoCatuara jointly failed to plead to the civil complaint within the
allocated time and are therefore in default.
6. Plaintiff hereby incorporates all prior pleadings in support of this affidavit and
motion.
