SUMMONS IN A CIVIL ACTION
SUPERINTENDENT SQUAREHEADDUDE:
A civil action has been brought concerning you, your business, or your office and docketed
before the Honorable T1dtony. A true and correct copy of the relevant Complaint, as it has been filed,
is attached to this Summons. You must act promptly to protect your rights.
Within five (5) days from the date you were served this Summons (excluding the day of
receipt), or within seven (7) days if you fall within the description provided in Rid. R. Civ. P. 4(d)(2),
you must appear in the above-named court and file an answer to the attached Complaint or furnish
another defense permitted by law or procedure. This time limit can be extended by presenting, in open
court, a request to the judge assigned to adjudicate this civil action.
An attorney can provide you with valuable legal assistance. If you wish to secure such
representation but cannot do so, the Ridgeway State Bar may, upon application, provide a referral, or
the above-named court may make an appointment.
If you fail to appear and file an answer or otherwise respond as required by this Summons, a
default judgment will be entered against you for the relief demanded in the attached Complaint.
