AFFIDAVIT FOR CIVIL DELINQUENCY STATUS
I, NationalLegislator, a Plaintiff in the above-styled case, have a bona fide belief that the
above Defendant is a delinquent in this court and hereby petition for this Court to declare them a delinquent
debtor under the Fugitive Debt Recovery Act of 2024. I made this petition based on the following beliefs.
The Superior Court for the State of Ridgeway, in a case styled as above, issued monetary judgment
against the Defendant upon order of the Court in the amount of $7,263 A copy of this judgment is attached
to this affidavit. I was authorized by the Court to collect said debt against the Defendant and such payment
was due on 08/14/2024.
I, or a person authorized by myself, reached out to the Defendant on 08/13/2024 to attempt to
collect the debt, informing them of the attempt to collect the debt. Defendant failed to make payment.
Based on the foregoing, your affiant submits that there is clear and convincing belief that the
above-listed Defendant is delinquent on their outstanding debt. Therefore, I request that the above-listed
Defendant be declared a delinquent debtor under the Fugitive Debt Recovery Act of 2024, and that all
conditions of delinquency status be imposed by the Court.

UPON CONSIDERATION of the Plaintiff’s Petition for Fugitive Status in the above-styled matter, it is
hereby ORDERED that the Petition be GRANTED and that the above Defendant is found to be delinquent
on a civil judgment levied against them under the Fugitive Debt Recovery Act of 2024.
