IN ACCORDANCE WITH RULE 37.
I, PrimaQuorom, solemly swear and affirm the following is the truth, the whole truth and
nothing but the truth to the best of my knowledge:
I filed the above entitled matter on 08/03/2022 and the filing fee at the courthouse was $125
which I paid.
I sought representation from 10 000 in connection with this case. They have served me with an
invoice which accounts for the costs and fees of this litigation to be paid to them in the total
amount of $1,220.00. Appendix A. I affirm that the appendix is a true and accurate
representation of the invoice, and further affirm that all of the fees are legitimate charges for
services that I received directly related to the litigation before this court.
