CIVIL SUMMONS
THE STATE OF RIDGEWAY TO THE ABOVE-NAMED DEFENDANTS:
RIDGEWAY COUNTY SHERIFF’S OFFICE, served through Sheriff Cephal as administrative
head pursuant to 7 R. Stat. Section 131.105; ERRCORE, in his official capacity; and AZAP634,
in his official capacity.
YOU ARE HEREBY SUMMONED and required to appear and answer the Complaint filed
against you in the above-captioned action.
1. A civil action has been commenced against you in the Superior Court of the State of
Ridgeway, County of Ridgeway, by the Plaintiff, Halo4asin. A copy of the Complaint, the
Civil Case Cover Sheet, the Plaintiff’s Exhibit List, and the Plaintiff’s pending motions is
served herewith.
2. You are required to serve upon the Plaintiff, and to file with the Clerk of this Court, a
written answer or other responsive pleading within seventy-two (72) hours after service
of this Summons upon you, or within such other period as the Ridgeway Rules of Civil
Procedure or an order of this Court may prescribe.
3. IF YOU FAIL TO RESPOND within the time stated, judgment by default may be entered
against you for the relief demanded in the Complaint, without further notice to you, and
the Plaintiff may proceed under the Ridgeway Rules of Civil Procedure and applicable
statute, including 7 R. Stat. Section 132.001 et seq.
4. The relief sought is declaratory and injunctive in nature. The Plaintiff seeks no monetary
damages.
5. This Summons is issued under the authority of the Superior Court of the State of
Ridgeway, a court of law and equity established under 4 R. Stat. Section 131.001, and is
directed to and executable by the Sheriff of the County of Ridgeway or any person
authorized by law to execute process pursuant to 7 R. Stat. Sections 131.101 and
131.102. Service may be effected by physical or electronic means pursuant to 7 R. Stat.
Section 131.103.
6. NOTICE OF CONFLICT IN SERVICE: Because the Ridgeway County Sheriff’s Office
is a named Defendant, and because its Court Liaison ordinarily effects service of process,
the Plaintiff requests that service upon the Defendants be effected directly, so as to avoid
having the Defendant agency serve process upon itself and its own officers.
