I, Detachment_Result, being duly sworn, do hereby state and affirm as follows:
1. I am the Plaintiff in the above-captioned matter. I make this affidavit based
on my personal knowledge.
2. The Defendant, ol1ievia, has failed to respond to the Complaint in this
matter.
3. The Defendant is indebted to me in the total amount of $9,500.
4. This amount represents the following:
a. $4,500 in punitive damages pursuant to 1 R. Stat. § 3113
b. $5,000 in punitive damages pursuant to 1 R. Stat. § 3109
5. To date, no part of the above amount has been paid by the Defendant.
6. I respectfully request that the court grant judgement in the amount of
$9,500, and any other relief the Court deems just and proper.
I declare under penalty of perjury that the foregoing is true and correct.
